Diseases with the same name do not have exactly the same course or treatment, and the more I look at the patients, the more I come across different aspects of the disease.
Physicians must identify the essence of each patient's disease and focus on the most appropriate treatment.
In addition, cutting-edge diagnostics provide physicians with unprecedented information, which can result in significant benefits for patients undergoing treatment.
Doctors must always keep an eye out for information on the latest medical equipment and diagnostic methods, and accurately determine whether or not they are truly useful.
I believe that disclosing information about one's own clinical experience and the testing methods that were used in the first place is an important task of doctors working in the medical field, even in the busy daily clinical practice.
Based on this idea, I have conducted clinical research and examined diagnostic methods on many themes, and published the results at academic conferences and in papers.
Here, I will briefly introduce the clinical research that I have conducted so far.

People who drink 3 or more cups of coffee a day are less likely to have a hidden cerebral infarction.
It is already known that dementia is more likely to develop when there is a hidden cerebral infarction.
It is important to know lifestyle habits that are less likely to cause hidden cerebral infarction.
As a result of investigating the causal relationship of hidden cerebral infarction by questionnaire and MRI of 242 brain checkup patients, it was found that people under 65 who drink 3 or more cups of coffee a day have less hidden cerebral infarction. and Cerebro Vascular Disease”

snowboarding injury

Snowboarding is a favorite winter sport, especially among young people, but it can cause life-threatening head injuries.
In the snowboarding boom before the Nagano Olympics, I thought it was an urgent task to investigate the characteristics of snowboarding injuries and how severe head injuries occur, so I conducted trauma research.
I conducted an epidemiological study of snowboard head injuries at a ski resort in the Suwa district. Severe head injuries during snowboarding are reported to occur when novices fall backward on a gentle slope and develop acute subdural hematoma, or when they fall while jumping.

I received my doctorate in medicine from the University of Tokyo for this research.Severe head injuries during snowboarding have been reported as a result of the reverse edge phenomenon during gentle slope gliding.JMS2002 97 snowboard mechanism.pdf
If you are familiar with this mechanism, you can prevent severe head injuries by using a helmet that protects the back of the head and by adopting a posture that immediately pulls your chin in the event of an injury.
These studies were published in the Journal of Trauma, the Journal of Neurosurgery, and the Journal of Neurosurgery.

Chronic subdural hematoma study
In older people, hematomas between the bone and the brain can develop after minor head injuries, which can be corrected with relatively simple surgery but are troublesome if they recur repeatedly.
In order to know the patterns of hematomas that tend to recur, and to consider surgical methods that are less likely to recur, I conducted clinical research and published two papers in the Journal of Neurosurgery and two in Neurosurgery.
We analyzed the natural history of chronic subdural hematoma and devised a new chronic subdural hematoma classification.JNS2001 95 CSDHclassification.pdf
Chronic subdural hematoma heals through four stages: homogeneous type, laminar type, separated type, and trabecular type. Since it serves as an indicator of the likelihood of recurrence, it has been adopted in neurosurgery textbooks (perioperative management of neurosurgery, 4th edition, Medical View, p.340, etc.) and cited in many papers worldwide. .
In addition, we announced in the "Journal of Nueurosurgery" that the recurrence rate after surgery is low when the tip of the hematoma cavity drain for chronic subdural hematoma is placed anteriorly. It is also quoted in neurosurgery textbooks (other than the above, neurosurgery revision 12th edition, Kinpodo p1927, etc.)JNS2000 93 CSDHdrainage.pdf
In addition, my thesis was also published in "Conservation Version Diagnosis, Treatment and Surgery of Chronic Subdural Hematoma (Medica Publishing 2017)" edited by Professor Kentaro Mori of the National Defense Medical College as a compilation of chronic subdural hematoma therapeutics. It is quoted in multiple places (p74,76,123,126,131,136).
In an analysis of 28 patients who underwent contrast-enhanced CT before surgery, we reported that the intimal enhancement increased as the hematoma stage progressed, and that the intima was strongly enhanced in 88% of patients with paralysis.CSDH病期と内膜造影性.pdf
We examined the hematoma component of chronic subdural hematoma and reported that hematoma hemoglobin, hematocrit, and plasma CRP (C-reactive protein) levels all decreased over time.

Calendar weather and stroke
I would like to investigate the specific days of hypertensive cerebral hemorrhage (days with a high incidence of cerebral hemorrhage according to the weather and calendar) and propose a way of life that prevents cerebral hemorrhage, such as avoiding exercise on the specific days.
We analyzed the interrelationship between hypertensive cerebral hemorrhage onset dates, calendar information, and meteorological information for a total of three years from 2001 to 2003 in the western region of Fukuyama City, Hiroshima Prefecture.
Multivariate analysis revealed that the average atmospheric pressure for the previous three days was above the yearly average (because temperature and atmospheric pressure are inversely correlated, the next day after three days of low temperatures), cerebral hemorrhages were common on holidays, and days when typhoons approached and Rokuyo were unlucky. I published the result that there are fewer people on days (butsumetsu, tomobiki) (presumably because there are no events on days when typhoons come or are avoided) in the "Journal of stroke and cerebrovascular disease" and "International journal of biometeology".JSCVD16 2007 ICH meteo calendar.pdf

MIB-1 and meningioma recurrence rate
Even meningioma, which is a benign tumor, can grow in a short period of time after surgery. We wondered if pathological examination of surgically obtained tumor tissue could be used to predict the rate of recurrence after surgery.
For that purpose, we investigated the correlation between the tumor doubling time (number of days until the tumor volume doubled on imaging) and the pathological examination (MIB-1 index) in 24 cases of recurrence after craniotomy for meningioma.
Demonstrating an inverse relationship between meningioma tumor doubling time and the MIB-1 index, published in Cancer 1999, one of the most important journals in oncology, It is still one of the reasons for using the MIB-1 index as an indicator of meningioma recurrence. This paper is cited with a figure in "Brain Tumor Therapeutics" (Kinpodo 2016), which was written as a culmination after retirement by Dr. Masao Matsutani, Professor Emeritus of Saitama Medical University, who has been a driving force in the world's brain tumor treatment for many years. increase. It is a great honor to have my paper selected as one of the 2130 carefully selected references from around the world in textbooks considered to be the bible of brain tumor treatment.MIB-1 & recurrent meningioma.pdf

Approach from the infratemporal fossa to the pterygoid process of the sphenoid sinus
Transpyramidal Cleft Approach to Cerebellar Vermis Hemangioma

I have devised an original (new) surgical approach and have written two papers.
In collaboration with the Department of Otolaryngology, the University of Tokyo, we devised a method of approaching the pterygoid process of the sphenoid sinus from the infratemporal fossa in the removal of giant meningioma from the middle cranial fossa, and published a paper on it (this was written by me). (This is my first paper.)蝶形骨洞翼状突起部へのアプローチ.pdf
I devised a post-pyramidal fissureectomy for vermis cavernous hemangioma, and published a paper detailing the surgical method.
経後錐体裂法.pdf
As mentioned above, it was reported that the recurrence rate after surgery is lower when the tip of the hematoma cavity drain for chronic subdural hematoma is placed forward, and this surgical method has become the world standard.

T2starWI/FLAIR法
We believe that the use of cutting-edge MRI and CT imaging methods is important for safe and accurate surgery, and we are focusing on neuroimaging research.
We used the FLAIR method, which was a new imaging method for MRI at the time, and T2* coordinated imaging, and reported its usefulness (3 papers in Japanese).

CTP CTA continuous imaging method
A CT examination method that performs cerebral blood flow examination and cerebral angiography at the same time was named the CTP/A method, and was jointly researched with GE, and reported its usefulness as a diagnostic method for hyperacute stroke (Japanese 3 papers, 2003).CTPA法.pdf

Other reports
In addition, I experienced noncommunicating hydrocephalus due to occlusion of the foramen Monroe in the softened brain after cerebral infarction, ganglioneuroma originating from the middle cranial fossa trigeminal nerve, and small cell carcinoma originating from the cavernous sinus (three articles).
